SERVICES DESCRIPTION
ASSESSMENT & RECOMMENDATIONS ($350):
Comprehensive assessment of communication skills focusing on specific areas based on client’s individual needs and goals. Pre-assessment telephone consultation. Post-assessment summary & written recommendations.
*At least one Follow-up visit is recommended to answer questions, monitor progress and provide additional recommendations & strategies.

FOLLOW-UP VISIT ($160 per visit):
In-person visit scheduled at the client’s convenience to review progress, answer questions and provide additional recommendations & strategies.

ASSESSMENT & CUSTOMIZED HOME PROGRAM ($960):
Comprehensive assessment of communication skills focusing on specific areas based on client’s individual needs and goals. Post-assessment summary & written recommendations. Customized & detailed home program based on client’s needs & goals.

Handouts, list of strategies, activities & recommended materials will be provided.
In-person demonstration of the strategies and activities to the individual who will be assisting the client with the home program (approximately one hour). One in-person follow-up visit is included in the Home Program package (approximately one hour).

ASSESSMENT & 6 TREATMENT SESSIONS ($1440):
Comprehensive assessment of communication skills focusing on specific areas based on client’s individual needs and goals. Pre-assessment telephone consultation. Post-assessment summary & written recommendations.

Six one-hour treatment sessions at the client’s home. Treatment will focus on specific goals based on the assessment results and the client’s individual needs. Treatment activities will target functional and meaningful skills to make the most impact in the client’s quality of life & daily activities. Treatment frequency will be determined based on the client’s needs & ability and may vary depending on client’s needs & progress. Post-treatment summary of progress and recommendations.

ASSESSMENT & 12 TREATMENT SESSIONS ($2400):
Comprehensive assessment of communication skills focusing on specific areas based on client’s individual needs and goals. Pre-assessment telephone consultation. Post-assessment summary & written recommendations.

Twelve one-hour treatment sessions at the client’s home. Treatment will focus on specific goals based on the assessment results and the client’s individual needs. Treatment activities will target functional and meaningful skills to make the most impact in the client’s quality of life & daily activities. Post-treatment summary of progress and recommendations.

BUSINESS HOURS: Typically Wednesday & Friday daytime
Mileage is included in the fee for clients who live in the Edmonton & St. Albert area. There may be an additional mileage fee for clients who live outside these areas.

Assessments are approximately one hour and are typically done at the client’s home.

There is an extra fee if a formal, detailed Assessment or Treatment Report is required.
